Health
We currently have two options for Health Care Providers. The first option is Blue Cross Blue Shield of Massachusetts, which is our option for a major medication plan. If a minor medical plan is more appropriate for your needs, you have the option to choose Starbridge Select, a CIGNA Healthcare product. All of the options and prices are listed below. Premiums provided below are deducted on a pretax basis in compliance with our Section 125 Cafeteria Plan.
Blue Cross Blue Shield:
HMO - Single: $411.71 a month = $190.02 per pay period
Family: $1235.14 a month = $570.06 per pay period
PPO - Single: $494.06 a month = $228.03 per pay period
Family: $1,482.17 a month = $684.08 per pay period
Starbridge:
Level 1 Single: $39.95 a month = $18.44 per pay period
Family: $147.51 a month = $68.08 per pay period
Level 2 Single: $72.76 a month = $33.58 per pay period
Family: $272.70 a month = $125.86 per pay period
Dental
Our dental program is administered by Delta Dental. The following two options are available. Premiums provided below are deducted on a pretax basis in compliance with our Section 125 Cafeteria Plan.
Single: $43.07 a month = $19.88 per pay period
Family: $118.54 a month = $54.71 per pay period
